The SomnoGuard AP2 & SPX are two-part, Mandibular Advancement Device comprised of a rigid outer tray and thermodynamic lining material enabling a Bite Registration in a simple chairside fitting process. The two piece format enables easy custom fitting and titratation, and permits significant freedom of lateral movement of the Mandible and mouth breathing during use to optimize comfort and therefore compliance. It can be refitted if necessary and has the ability to advance the mandible by up to 10mm when needed. It can be used in conjunction with both nasal and full face mask CPAP therapy as well to substantially lower CPAP pressure settings.

The SomnoGuard line of Oral Appliances are 510(k) cleared Prescription Devices, making them eligible for coverage under the majority of commercial insurance plans that recognize Pre-Fabricated, Custom-Fitted Oral Appliances (HCPCS E0485) as Medically Necessary for patient Diagnosed with Obstructive Sleep Apnea.
Titration of the device should be managed by the physician to optimize efficacy while limiting the potential for complications. Complications from excessive adjustment can include but are not limited to dental movement, TMJ and muscle soreness and non-compliance due to discomfort. Other potential complications that have been reported include Hypersalivation (transitory), loss or damage of the device, Nausea, Tooth Ache, a suffocation or clasutrophobic sensation, breathing problems and dental complications that rarely present but can be incidental to the use of any oral appliance. Initial advancement of the Mandibular portion of the device is recommended to be no more than 30% of the total anterior movement ability of the specific patient's mandible and is highly recommended to be performed gradually, no more than 1mm or week until subjective efficacy is achieved. Any dental abnormalities should be addressed prior to fitting and initiation of Oral Appliance Therapy by referral to a dentist. Poor Oral Hygiene or dentitia issues typically require a Custom Fabricated device for proper retention and are not appropriate for use of the SomnoGuard AP2 or SPX. 1st Line Medical provides the SomnoGuard AP Pro for patients in need of a Custom Fabricated appliance. Dentures, peridontal disease, loose teeth or hardware including temporary crowns, fittings, etc. are explicit contraindications to the fitting and use of the SomnoGuard.

The two piece design is comprised of a solid housing which contains a thermodynamic material that enables efficient and effective registration by heating the material, thereby softening it, and then cooling the fitted device to maintain the retention.
