The Prescription Service Program enables you to immediately implement a comprehensive sleep practice by completing and faxing the following Certificate of Medical Necessity form, along with the patient's insurance information, to our facility. We will then coordinate the service with the insurer and patient, relieving you and your staff of this administrative burden. We program, charge and configure the WatchPAT unit and deliver it to your patient's home via UPS with Instructional Brochure and 24/7 toll-free tech support. Upon completion of the study, the patient conveniently returns the device with a provided, prepaid Shipping Label. The Data is downloaded, confirmed sufficient for diagnosis, and interpreted by a Board Certified Sleep Physician and forwarded to you.
